Parathyroid cysts usually arise secondary to the cystic degeneration of parathyroid adenomas. Dutta and colleagues present the case of a 24-year-old female with the clinical and biochemical features of primary hyperparathyroidism (PHPT), intact parathyroid hormone (iPTH), and poor radiotracer uptake. The report highlights the importance of FNA-iPTH in localizing and differentiating a functional parathyroid lesion from nonfunctional tissue in PHPT.
